Thank you for registering your interest to partake in our CHE Live Student Clinic

• The CHE Live Student Clinic provides patients with affordable care, while providing a valuable learning experience to our students.
• All Live Clinics are conducted through the Zoom platform  

• Patients interested in the Live Clinic, need to commit to 4 x monthly appointments at $50 each (remedy costs are additional) within the Live Clinic setting.
• If treatment is still needed after this time, you will have the opportunity to become a private patient of the practitioner.
• If 2 appointments are missed, postponed or cancelled we will no longer be able to treat you in clinic.

• Upon acceptance into the Live Clinic, you will be required to sign a document agreeing to have your case recorded and used for teaching purposes, through our secure learning platform. Your information will always be handled and respected with the strictest confidentiality.
